ATTENDANCE CONTRACT
You may print and use this contract in your meeting with the student and family to discuss attendance concerns. School staff must document that attempts have been made to involve the student and family in an agreement to address attendance. Signatures are required.

SAMPLE 3-DAY LETTER
Continuing truant letter can be used or modified to fit the needs of your school. If needed in other languages, please contact our office at (612) 348-6041.

SAMPLE LEGAL WITHDRAWAL
Here is a sample form to complete with a parent and student when the student is 16 or 17 years old and the parent wishes to withdraw the student from school. You may adapt this form as necessary to comply with any additional requirements of your school.
